Professional Requirements

For legal and ethical reasons, professional account holders must supply verification of professional status. Therapists and counselors must fax our office their certification identification. Clergy must provide proof of their ministerial card. Professors need to provide proof of their status at a university or college. Please call the RELATE Institute for more information at (801) 422-4359 or fax your documentation to (801) 422-0225. You must also complete registration information.

In order to apply for a professional account, please follow these steps:

1. Create a standard RELATE account (if you do not already have one)
2. Complete the online professional application
